 Register at Milton Medical Practice

If you think you would like to join our practice as a patient then please contact us and we can discuss it with you or - alternativly - drop in to our reception desk and we can arrange things for you from there.

·         Please bring your NHS medical card and Birth certificate with you

·         If you do not have a medical card you will be asked to complete a registration form for each person registering.

·         You will be asked for proof of address (eg utility bill)

·         You will be asked to complete a new patient questionnaire, allowing us to provide medical care in the interim period, while your medical records are transferred from your old practice.

·         We will make an appointment for you to have a new patient health check with the Practice Nurse. 

·         We will make arrangements for your health records to be transferred to us from your current GP
